1. Agreement to Terms
Welcome to The One Spa Chicago! These Terms and Conditions (“Terms”) constitute a legally binding agreement made between you, whether personally or on behalf of an entity (“you”) and Streeterville ArtMed, LLC, doing business as The One Spa Chicago (“The One Spa Chicago,” “we,” “us,” or “our”), concerning your access to and use of the https://theonespachicago.com/ website (the “Site”) as well as any related services, including appointments booked and services rendered at our physical location at 432 E Grand Avenue, Suite D, Chicago, IL 60611.
You agree that by accessing the Site and/or using our services, you have read, understood, and agree to be bound by all of these Terms. IF YOU DO NOT AGREE WITH ALL OF THESE TERMS, THEN YOU ARE EXPRESSLY PROHIBITED FROM USING THE SITE AND OUR SERVICES AND YOU MUST DISCONTINUE USE IMMEDIATELY.

4. Appointments and Bookings
Appointments can be booked through our Site, by phone (312-340-3740), or via email (Schedule@TheOneSpaChicago.com). All appointments are subject to availability. We will send confirmation of your booking, typically via email or SMS text message if you have consented to receive such messages.
5. Payment Terms
Prices for services are listed on our Site or available upon request and are subject to change without notice. Payment for services is due at the time the service is rendered unless otherwise agreed upon. We accept various forms of payment, which will be specified at the time of booking or payment. All payments are processed securely, often using third-party payment processors. You agree to provide current, complete, and accurate purchase and account information. Applicable taxes will be added to the service fees.
6. Cancellation and Rescheduling Policy
We understand that circumstances may require you to cancel or reschedule your appointment. We request that you provide at least 24 hours’ notice prior to your scheduled appointment time.
- Cancellations/Reschedules with 24+ hours’ notice: No charge.
- Cancellations/Reschedules with less than 24 hours’ notice or No-Shows: We reserve the right to charge a cancellation fee [Optional: Specify Fee Amount or Percentage, e.g., “equal to 50% of the scheduled service value” or “a flat fee of $XX”] or require pre-payment for future bookings.

7. Client Responsibilities
- Timeliness: Please arrive on time for your appointment to ensure you receive your full service. Late arrivals may result in a shortened service time to accommodate subsequent appointments, potentially at the full service price.
- Health Information: Your safety is important. You agree to provide accurate and complete information about your health conditions, allergies, medications, and any other relevant factors prior to receiving any services. This information allows us to tailor services safely and effectively. Failure to disclose relevant information may result in harm for which The One Spa Chicago cannot be held liable.
- Conduct: We strive to provide a relaxing and professional environment. We reserve the right to refuse or discontinue service to anyone demonstrating inappropriate behavior towards our staff or other clients.
8. Disclaimers
- Service Results Disclaimer: Spa services provide aesthetic and wellness benefits, but results can vary based on individual factors. We do not guarantee specific results from any treatment.
- Website Content Disclaimer: The information provided on the Site is for general informational purposes only. You agree to defend, indemnify, and hold us harmless, including our subsidiaries, affiliates, and all of our respective officers, agents, partners, and employees, from and against any loss, damage, liability, claim, or demand, including reasonable attorneys’ fees and expenses, made by any third party due to or arising out of: (1) your use of the Site or Services; (2) your breach of these Terms; (3) any breach of your representations and warranties set forth in these Terms; or (4) your violation of the rights of a third party.

These Terms and your use of the Site and Services are governed by and construed in accordance with the laws of the State of Illinois applicable to agreements made and to be entirely performed within the State of Illinois, without regard to its conflict of law principles.
12. Dispute Resolution
Any legal action of whatever nature brought by either you or us (collectively, the “Parties” and individually, a “Party”) shall be commenced or prosecuted in the state and federal courts located in Cook County, Illinois, and the Parties hereby consent to, and waive all defenses of lack of personal jurisdiction and forum non conveniens with respect to venue and jurisdiction in such state and federal courts. Application of the United Nations Convention on Contracts for the International Sale of Goods is excluded from these Terms. 15. Miscellaneous

Our failure to exercise or enforce any right or provision of these Terms shall not operate as a waiver of such right or provision. If any provision or part of a provision of these Terms is determined to be unlawful, void, or unenforceable, that provision or part of the provision is deemed severable from these Terms and does not affect the validity and enforceability of any remaining provisions.
